It was reported that a patient death occurred.A 10mmx3.25mm wolverine coronary cutting balloon was selected for use in the mid to proximal right coronary artery.During the procedure, following pre dilatation, intravascular ultrasound (ivus) was then performed.The balloon catheter was pressurized at 6 atmospheres, 12 atmospheres, and 20 atmospheres, then a guide extension catheter was advanced, while the balloon was deflated, to capture the distal anatomy and to deliver the stent.After removing the balloon catheter, the physician injected contrast, and perforation was noted.Another balloon catheter was advanced until a covert stent was able to be opened and prepped for delivery.However, the patient was noted to have a large effusion and passed away eleven days following the procedure.
