|
It was reported that during a cori-assisted ukr, the ri robotic drill attachment white sealing tip was missing.Surgery was performed, without any delay, with a back-up device.No patient complications were reported.

The cori drill attachment rob10015, (b)(6), used during treatment, was returned for evaluation.The external condition shows signs of moderate wear, including a missing wiper.The reported scenario that the wiper is missing can be visually confirmed.The most likely cause of this event is normal wear and tear from use over time loosening the adhesion of the wiper.A review of manufacturing and service records indicate the device met all specifications upon release into distribution.A complaint history review for similar reported/confirmed complaints found similar events.A historical escalation event review was completed and found no prior escalation actions applicable to the scope of the reported complaint.Although no further containment or corrective action is recommended or required at this time, the failure mode will continue to be closely monitored through complaint investigation and trended through post market surveillance activities.Internal complaint reference number: case-(b)(4).
