Event Description
It was reported that the during support, a faulty pcb assembly and a re-circulating water problem were found.No patient was involved.

B3: date of event is unknown, no information has been provided to date.Investigation including root cause analysis is in progress.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.

Other text: d10.Device available for evaluation; h3.Device evaluated by manufacturer and h6.Evaluation codes: updated.One device was received.Visual inspection noted the return tube was cracked and the printed circuit board (pcb) had evidence of fluid ingression.Functional testing confirmed the complaint as the device was not entering "operation mode" (green led on display is not lit) it powered on but would not run.The root cause was due to a cracked return tube which allowed fluid to leak onto pcb causing it to fail from a design issue.The product's history records were reviewed and there were no non-conformances nor service-related issues that would have resulted in the reported complaint.Replaced the return tube and pcb to correct the reported problem.Replaced the o-rings per preventative maintenance (pm).Device passed all functional and delivery tests.
