Event Description
Medtronic received information regarding a navigation device being used during a sacroiliac and thoracolumbar procedure.It was reported that the driver was not engaging the screw properly causing the screws to load off axis (crooked).The screw tip would not be straight when engaged with the driver and the screw tip remained loose.A different driver was used and the issue resolved indicating that the issue lay with the screw driver and not the screws.There was less than an hour delay to the case.No reported impact to the patient.
 
Manufacturer Narrative
