The device was returned to olympus for evaluation and the customer's allegations were partially confirmed.There was an intermittent grinding detected on the forceps elevator lever, however, there were no issues found concerning the air/water flow.Other evaluation findings are as follows: there was a cut on switch#1; the play of the upward/downward knob was not within standard value; there was a gap on the plastic distal end cover; the objective lens glue was old; there was a chip on the light guide lens; and the bending section cover glue was cracked and discolored.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

It was reported that the insufflation was not working and the elevator was sticking.These events were found during preparation for use.There was no report of patient harm.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to rust that was caused by gradual scraping of the metal part of the l-arm (forceps elevator) which generated scraped metallic powder around the arm, which then rusted from moisture in the air.As there was no information received of clear misuse of the device, there is no labeling advisory that was deemed required or recommended for the user facility.It is suggested that the user facility review the method of handling of the device in accordance with the instructions for use (ifu).Olympus will continue to monitor field performance for this device.
