Event Description
|
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving morphine of an unknown concentration at an unknown dose rate via an implantable pump.It was reported that the healthcare provider was unable to read the patient's pump.It was further reported that the patient stated that it felt like the pump was moving around.The date of the event was unknown.The pump was very difficult to palpate.The physician confirmed that the pump was flipped under fluoro.The physician flipped the pump over in order to refill the pump and was sending the patient back to an alternate physician for a pocket revision.Regarding factors that may have led or contributed to the issue, pump placement was indicated.The issue was not resolved as of (b)(6) 2023.The patient was without injury regarding their status as of (b)(6) 2023.It was unknown if surgical intervention was planned.The patient's medical history and weight at the time of the event was unknown.

Additional information was received from a healthcare provider (hcp) via the manufacturer representative (rep).It was reported that the patient had a pocket revision today.The pump was rea-anchored to the fascia and repositioned to help prevent further movement and flipping.

Additional information was received from a healthcare provider via a manufacturer representative regarding a patient receiving unknown morphine (4mg at 1.1mg/day) via an implantable pump.It was reported that the patient was in to have a refill under fluoro.It was found that the pump was flipped again after the revision.A pocket revision was performed on (b)(6) 2023 to anchor the pump due to excessive movement and flipping after the initial implant.The patient stated that they did not feel it flip this time as they did prior to the pocket revision.The hcp flipped the pump back over to complete the refill.The patient would continue to have refills under fluoro and will go back to the surgeon if the pump continued to flip.The issue was resolved at the time of the report.It was noted that the hcp had no further information.The patient's weight and medical history were asked but would not be made available.No patient symptoms were reported.The patient status at the time of the report was alive with no injury.
