The device was returned to olympus for evaluation and the customer's allegation was not confirmed.The device evaluation found blood on the biopsy channel connection in the connector.In addition to the reportable malfunction, the following were noted: the jet-channel was perforated; the charged coupled device cover lens was chipped; the plastic distal end cover was chipped; the bending tube was shortened; the connecting tube had a scratch; the suction cylinder nut had paint peeling off; the plug unit was cracked; the angulation and the play of the upward/downward and right/left angulation knob were out of the standard values; and there was no brake engagement during angulation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported to olympus that the evis exera iii gastrointestinal videoscope had a broken wire.The issue was found during reprocessing.The device was returned for evaluation.During the device evaluation, the following reportable malfunction was found: blood on the biopsy channel connection in the connector.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
Correction to g2, health professional was inadvertently not selected in the initial report.This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a root cause could not be determined.There was no physical damage at the point the material remained.Despite multiple good faith attempts, olympus could not obtain additional information.Therefore, it was unknown whether there was deviation from instructions for use (ifu) on reprocessing steps at the point the material remained.The suggested event can be detected/prevented by handling the device according to the following ifu.Detection methods are written in ifu: gif/cf/pcf-190 series operation manual chapter 3 preparation and inspection.Prevention measures are written in ifu: gif/cf/pcf-190 series reprocessing manual chapter 5 reprocessing the endoscope.Olympus will continue to monitor field performance for this device.
