It was reported that a contamination occurred.A rotawire drive was selected for use.When the device was taken out of the hoop, it was noted that a rough substance was attached at the bottom part of the wire.The device was not used inserted inside the patient.The procedure was completed with another of the same device.There were no patient complications reported.

Manufacturer Narrative
Device evaluated by mfr: the device was returned for evaluation.Microscope inspection revealed that the spring tip was detached.No other issues were identified during the product analysis.
