Event Description
It was reported that during a primary total hip arthroplasty the actis broach would not fit into the kincise anterior adapter.No surgical delay.

Product complaint # (b)(4).Investigation summary an analysis of the product could not be performed since a physical sample was not received for evaluation.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of the post-market surveillance.However, if the product is received at a later date, the investigation will be updated as applicable.

Additional information received.A.Did the instrument broke into two or more pieces? if yes, were the fragments removed from the patient? the broach did not break into pieces.B.Why won't the size 8 actis broach would not fit into the kincise anterior adapter? exact allegation/malfunction of the broach.The proximal attachment point for the broach with the handle was warped somehow.
