Information was received from healthcare provider (hcp) via a manufacturer representative regarding a patient having screw replacement due to pjk (proximal junction kyphosis).Pre op diagnosis was proximal junction kyphosis.It was reported that the tip of the screwdriver broke when the screw is replaced.Screws were loose and patient had incomplete fusion and proximal junction kyphosis as a result.There was loosening in the screws on both sides of l2, on the right l3, on both sides of l5, and on both sides of s1 (a total of 7 screws), so all spinal screws were replaced.There were no further complications reported regarding the event.
