Event Description
Medtronic received information regarding a catheter.It was reported that during a placement of the catheter while using the device to create a tunnel for draining.The tip of the device couldn¿t be found so it was left inside the patient's body.The health care professional (hcp) was unable to ¿push¿ the passer through due to a lot of resistance.The hcp stated that this was not uncommon.When pulling back the passer the hcp saw the tip of the device was missing/broken off.The hcp tried to find the tip by palpating and echography but was not able to find it.They made the decision to leave it in the patient's body.There was a 60 minute delay to the case.

Additional information was received.There was no other reported impact to the patient.
