Event Description
The customer reported to olympus that when using a telescope, 30°, wideangle, 4 mm, the image was blurry.The procedure was completed with the same device.There was no procedural delay, or no patient harm associated with the device.The device was returned and evaluated, and upon estimation, the lens was damaged with displacement, the distal end was burnt and worn, and the direction pin was damaged.This mdr is being submitted to capture the reportable malfunction found during the device evaluation.
 
Manufacturer Narrative
The device was returned and evaluated, and the customer¿s allegation (blurry image) was confirmed.In addition to the damaged lens with displacement, the burnt and worn distal end, and the damaged direction pin, additional evaluation findings were as follows: the outer tube was skewed, and the label of the eyepiece was worn.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the cause of the event is likely due to excessive force by the customer.Olympus will continue to monitor field performance for this device.
