A facility reported that the torque screw on mayfield infinity skull clamp (a1114) does not exert pressure, and the force indicator of the adjustment screw does not allow to visualize the value of the force applied in increments of 20 lbs.The failure occurred in surgery with a patient, which caused an adverse event.However, no surgical delay nor patient injury reported.

The mayfield infinity skull clamp (a1114) was not returned for evaluation; therefore, an evaluation of the device could not be performed.Lot number information has been provided; therefore, device history record (dhr) was reviewed, and no anomalies were found.The root cause of the reported issue could not be determined; however, based on the reported complaint, probable root cause is damage to the torque screw or an out of specification hastalloy spring.If additional relevant information becomes available in the future, this complaint will be reopened, and the respective evaluation performed.Trends will be monitored for this and similar issues.At present, we consider this complaint to be closed.
