It was reported that the impactor threads seem to be worn therefore not engaging in the cup's threads.No debris fell in the patient nor was left in the patient.There is no additional information available at the time of this report.

This follow-up report is being submitted to relay additional information.Visual examination of the provided pictures identified the thread damage is unable to be confirmed.The handle shows signs of wear and tear.Unable to confirm complaint as the product was not returned and the image provided does not show the reported damage.Lot identification is necessary for review of device history records, lot identification was not provided.Medical records were not provided.A definitive root cause cannot be determined if any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
