Event Description
The customer reported to olympus that the cysto-nephro videoscope had an air leak.Upon inspection and evaluation, scraping of the forceps stopper cap.There was no report of patient harm or user injury associated with this event.This medical device report (mdr) is being submitted to capture the reportable malfunction found during incoming inspection and evaluation.
 
Manufacturer Narrative
The device was returned to olympus for inspection, and the customer's issue of ¿air leak" was confirmed.The following findings were noted during device evaluation: insufficient bending angle, resin float, insertion tube collapse, scratches and wrinkles, universal cord crushed and scratched, video cable crushed and scratched, rattling at the connection between the curved tube and the tip, scratches on curved rubber joints, video connector scratches, light guide connector scratches, video connector case scratches, operation part scratch, switch box scratch, operation part cover scratch, grip scratch, up/down scratch, up/down plate scratch, up/down angle fixing lever scratch, and vent metal misalignment.The investigation is ongoing, and follow-up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the instrument channel port was confirmed to be scraped.However, the definitive root cause of the scraped instrument channel port could not be determined.Olympus will continue to monitor field performance for this device.
