According to the reporter, during the laparoscopic distal pancreatectomy, while transecting the pancreas, the stapler fired completely through the reload but the jaws of the device locked on tissue. the surgeon was able to remove the reload by exerting excessive force on the black knob on the handle which disrupted the staple line on the pancreas.
