At this time, it has been indicated to olympus that the device will not be returned for testing and evaluation.Should additional information become available, or the evaluation is completed, a follow-up report will be submitted.
 
Event Description
The customer reported to olympus the evis exera ii ultrasound gastrovideoscope was returned from repair and noticed that the scope had transferred lettering from the olympus logo print from the plastic wrapping.The reported issue was discovered upon initial receipt on the returned device.The facility proceeded to complete ¿multiple¿ cleaning sterilization and disinfection (cds) processes to remove the blue stain/dye.There was no patient/user harm or injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation and correction to d8.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the ink transfer was the endoscope must have been wet with water or alcohol; therefore, the letters of the plastic wrapping were transferred onto the endoscope.The cause of water or alcohol residue on the device could not be identified.Olympus will continue to monitor field performance for this device.
