Event Description
The customer reported that a wire stent broken and was lodged in the evis exera iii duodenovideoscope.The issue occurred during a procedure.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation was not confirmed.The device evaluation found the following: plastic distal end cover and insertion tube insulation resistance value was low, the bending section cover was cracked, and the insertion tube had scratch marks.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause was unable to be determined.The event can be prevented by following the instructions for use (ifu): (operation manual) 3.2 inspection of the endoscope.Olympus will continue to monitor field performance for this device.

The diagnostic procedure was not prolonged at all, as the issue occurred during cleaning after the case.
