It was reported that on an unknown date, while resetting trays the sales rep found issues with the instruments in question.The tip of the depth gauge was found to be bent, the quick connect screwdriver would not engage with driver shafts, and the quick connect handle was broken.There was no patient or procedure involvement.No additional information is available.This report is for a handle with mini quick coupling.This is report 1 of 3 for (b)(4).
