Event Description
A physician was implanting an everflex entrust for the treatment of a 120mm calcified lesion in the superficial femoral artery.The artery was 6mm in diameter with moderate tortuosity.The device was prepped as per the ifu with no issues identified.A 6fr sheath was used.The device had not passed through a previously deployed stent.It was reported that the system was deployed for 75% and the thumbwheel got stuck.The length of the deployed stent in the vessel was 150mm.Stent dislodgement occurred.The dislodged stent was not removed, after it flowered it was not repositioned.The deployment catheter was removed and the stent was elongated.The physician completed the procedure, an unknown stent was used after the medtronic stent was deployed.No patient injury was reported.
