Event Description
The customer reported to olympus that the evis exera iii duodenovideoscope was in use for a unspecified procedure.The customer reported stent got stuck and could not be removed from the device.The customer confirmed the procedure was not prolonged and there was no extension of patient sedation.No adverse effects to patient reported.

The device has not been returned for evaluation.If the device is returned, an investigation will be performed and a supplemental report will be filed.Investigation is ongoing.This report will be supplemented accordingly following investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.The event can be detected by following the instructions for use (ifu) section: operation manual 3.3 inspection of the endoscope.The event can be detected and prevented by following the instructions for use (ifu) section: reprocessing manual 5.5 manually cleaning the endoscope and accessories.Olympus will continue to monitor field performance for this device.
