Event Description
It was reported that the procedure was to treat a heavily calcified lilac artery.The 7x40mm armada 35 balloon dilatation catheter (bdc) was advanced with a 7f catheter once at the lesion the balloon was inflated at 6 atmosphere and ruptured.It was not possible to retrieve the balloon threw the catheter as several attempts were made.The balloon with the guiding catheter was removed via cut down.It was noted during the cut down the balloon had to be cut into separate pieces in order to remove the device.There was no adverse patient sequela; however, clinically significant delay was noted.No additional information was provided.
 
Manufacturer Narrative
Visual inspection was performed on the returned device.The reported balloon rupture was confirmed.The reported entrapment of device could not be replicated in a testing environment as it was based on operational circumstances.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents and/or complaints from this lot.The investigation determined the reported balloon rupture, entrapment of device, surgical intervention and delay to treatment/therapy appear to be related to circumstances of the procedure.There is no indication of a product quality issue with respect to design, manufacture or labeling of the device.
