Event Description
It was reported that noise was observed on the right ventricular channel.Reprogramming options were discussed.During a generator changeout procedure, the left ventricular (lv) lead was placed into the right ventricular port and loss of capture (loc) was noted.  different system set up options were discussed.The lv lead remains in service.No patient complications have been reported as a result of this event.
