Event Description
Patient slipped in shower at home and fell (b)(6) 2023.Because of the fall the patient's incurred a traumatic hip dislocation.Attending to emergency room physician was able to relocate the hip to the acetabulum, but upon post reduction radiograph it appeared the bipolar shell had disassociated from the 28mm ball which was still on stem.Bipolar shell was floating around the inner capsule area, creating an unstable hip construct.Patient was admitted as inpatient on (b)(6) 2023 and surgery open reduction/hip revision scheduled for (b)(6) 2023.Revision surgery revealed that the 28mm ball disassociated from the poly liner, but the poly liner did not dissociate from the outer metal shell.A new 28mm of the same neck length and self-centering bipolar shell was presented and implanted.The hip was then reduced without incident and was found to be stable.Original surgeon would like for implants to be reviewed.Doi: (b)(6) 2023.Dor: (b)(6) 2023.Affected side: right hip.
 
Event Description
Additional information was received and stated that according to the surgeon, the disassociation came from the er physician trying to relocate the hip.

Product complaint # (b)(4).Investigation summary : an analysis of the product could not be performed since a physical sample was not received for evaluation.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of the post-market surveillance.However, if the product is received at a later date, the investigation will be updated as applicable.

Product complaint #(b)(4).Investigation summary: no device associated with this report was received for examination.In order to determine if a lot related issue was possible, a worldwide complaint database search was performed.A worldwide complaint database search found no additional related reports against the provided product code/lot code combination.Based on the inability to find any additional related reports against the provided product code/lot code combination it is reasonable to conclude that there are no anomalies with regard to manufacturing or inspection contained in the device history records that would contribute to the reported event.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of the post-market surveillance.If additional information is made available, the investigation will be updated as applicable.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
