Event Description
The olympus distributor reported (on behalf of the customer) that while using the visera elite xenon light source, the emergency lamp indicator and the front panel were blinking.The issue was found during inspection for use before a therapeutic procedure.The procedure was completed with a similar device and without delay.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to olympus for evaluation, and the customer's allegation was confirmed.The device evaluation found that the emergency lamp burned-out panel was flashing.The investigation is ongoing, and a supplemental report will be submitted upon completion or if the user facility provides any additional information.

This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: a review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over four (4) years since the subject device was manufactured.A definitive root cause could not be determined.Based on the results of the investigation, it is likely the following led to the malfunction: the cause was the faulty emergency lamp.Olympus could not surmise the cause of the defect.Olympus will continue to monitor field performance for this device.
