It was reported that the patient experienced chest pain and atrial fibrillation (af) and received five inappropriate shocks by the cardiac resynchronization therapy defibrillator (crt-d) device resulting in hospitalization.Interrogation of the device revealed detection of atrial fibrillation (af) with rapid ventricular response. Some of the shocks were able to convert the af into sinus rhythm for a short time before af recurred. When the patient presented to the emergency department they were noted to be in sinus rhythm. An magnetic resonance imaging (mri) was performed and revealed a pericardial effusion and pericardial enhancement indicating pericarditis. The patient was hospitalized, reprogramming was performed, and the patient was treated with an antiarrhythmic, blood thinner and an intravenous saline bolus. The device is still in use.The patient is a participant in a clinical study. No further patient complications have been reported as a result of this event.
