It was reported that the device was not powering on.The issue was discovered during preventative maintenance.No harm or adverse effects were reported.

Other text: b3: date of event and d4: udi section are unknown, no information has been provided to date.Investigation including root cause analysis is in progress.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.

Device evaluation: one device was returned for investigation.Visual inspection noted: device received with a corroded quick connect, water tank cover cracked on all four corners, plate clip discolored, line cord faded and worn, pole clamp had gouges in the center of holder, front cover was scratched, and the reflux plug was missing.Functional testing confirmed the complaint when the device would not turn on.Root cause was attributed to a faulty printed circuit board (pcb).The product's history records were reviewed and there were no non-conformances nor service-related issues that would have resulted in the reported complaint.Replaced pcb, quick connect, water tank cover, plate clip, line cord, pole clamp, reflux plug, and front cover.Performed preventative maintenance.Changed o rings and reservoir gasket.Calibrated device.Device passed all functional testing after the completed repairs.
