The mayfield modified skull clamp (a1059) was returned for evaluation: failure analysis - the investigation of the returned device showed that the skull clamp had a sticking swivel lock, and opening the mechanism showed that the base casting¿s plain bearing surface had dents.The base casting was replaced along with the internal parts to adjust the swivel lock assembly.After completing the repair, a successful functional test was performed.Root cause - evaluation found no device deficiencies that would have contributed to the reported complaint.The swivel lock was sticking and required replacement of worn internal parts; however, this would not have contributed to patient slippage.Probable root cause is improper or suboptimal placement of the skull clamp on the patient.No further investigation required based on the acceptability of risk and no adverse trends identified.This will be monitored and trended going forward.At present, we consider this complaint to be closed.
