Event Description
According to the reporter, during laparoscopic digestive surgery, the clip applier frayed the trocar (different manufacturer) when inserted into the cavity.As a result, a 2mm plastic material of the trocar fell against the cystic duct.The piece was retrieved.There was no patient injury.
 
Manufacturer Narrative
