The device was returned to olympus for evaluation where service confirmed the customer's complaint.Additionally, it was found that there was peeling of the coating on the emergency light harness and the high brightness switch turned off due to damage to output connector.The investigation is ongoing; therefore, a root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus that the visera elite xenon light source lamp does not turn on and the secondary emergency light does.The reported problem was found during preparation for an unspecified therapeutic procedure.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eleven (11) years since the subject device was manufactured.Based on the results of the investigation, it is not possible to establish the root cause.However, it is likely that the main light did not turn on and the emergency light turned on due to the failure of the igniter.Olympus will continue to monitor field performance for this device.
