Event Description
According to the reporter, when the product package was opened during surgery, the metal tunnel needle included in the kit was broken.Besides the reported issue, there were no other visible defects/damages found on the product.There was no leak.There was no luer adapter issue.Tego was not utilized, and there were no other products utilized with the device.There was no excessive force used on the device.There was no cleaning agent used on the device.The catheter was not repaired.There was no remedial action done, and the product was not replaced.The procedure was completed.There was no blood loss due to the event.There was no intervention/treatment required due to the event.There was no reported patient injury.

Event Description
According to the reporter, when the product package was opened during surgery, the metal tunnel needle (tunneler) included in the kit was found to be broken.It was stated that the issue was found after opening the package and not during performing the operation.Besides the reported issue, there were no other visible defects/damages found on the product.There was no leak.There was no luer adapter issue.Flushing was not performed.Tego was not utilized, and there were no other products utilized with the device.There was no excessive force used on the device.There was no cleaning agent used on the device.The catheter was not repaired.The reported tunneler was not used.A new device was used to resolve the issue and to complete the procedure on the same day of the event.The procedure was completed.There was no blood loss due to the event, and a blood transfusion was not required.There was no intervention/treatment required due to the event.There was no reported patient injury.
