Event Description
|
A physician was attempting to implant an everflex entrust for treatment of a calcified lesion with 80% stenosis in the distal region of the left superficial femoral artery.The artery was moderately calcified and tortuous.The device was prepped as per the ifu was no issues identified.It was reported that resistance was met during delivery to the lesion.The device passed through a previously deployed stent.The lock pin was checked for securement prior to the procedure and removed just prior to deployment.The clam shell was cracked open, retraction string was gripped but frayed off.The partially deployed stent was pulled back into the sheath was removed and exchanged for new sheath.No patient injury was reported.
