The device was returned and evaluated.The customer¿s allegation was confirmed and the e207 error was displayed.No other abnormalities were found.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus they received an e207 error code during pre-use inspection of the visera elite xenon light source.The customer indicated there was no problem with the emergency lights, so the therapeutic procedure was not affected.The procedure was completed using the same equipment.There was no reported patient injury or medical intervention associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Based on the results of the investigation, the emergency light indication flashed and an e207 fault has been confirmed.Failure of the emergency light and tortuosity of the lead pin were identified.It was probable that an irregular external force was applied to the lead pin causing damage to the lead pin connection or causing it to short-circuit.The asset was replaced with an emergency light, and the issue was resolved.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 2 years since the subject device was manufactured.Olympus will continue to monitor field performance for this device.
