Event Description

It was reported that extension was identified as the cause of oor intra-op impedances. the surgeon said that he and/or his surgeon c olleague "over-stretched" and damaged the extension when attempting to implant it. did multiple intra-op troubleshoots: tested system impedances, tested lead impedances to rule it out, and swapped ports on generator to rule out generator as source of the problem.The extension was replaced and the issue was resolved.No symptoms reported.

Additional information received from the manufacturer¿s representative (rep), which was confirmed with the healthcare provider (hcp), reported they felt the impedance issue was due to something done by the surgery team, but this couldn¿t be confirmed.The extension was replaced with a good outcome.
