Event Description
The customer reported the manual brightness of the evis exera iii xenon light source would not get brighter.The issue was found during a diagnostic laparoscopic procedure that took an hour to complete.No error messages were displayed and just the view had dim light.The setup was noted to be normal and no other devices were connected besides the camera head and cord.The case was completed with a similar device.
 
Manufacturer Narrative
In speaking with olympus technical assistance center (tac) the customer tried unplugging and reseating both of the light cables to troubleshoot the issue.Reseating the cables resolved the issue and the customer confirmed that manual and auto brightness were working.This event is under investigation.A supplemental report will be submitted upon receiving additional information.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over six (6) years since the subject device was manufactured.Based on the results of the investigation, the root cause of the problem could not be identified, the probable cause: a human error made at the time the customer operated the light source.Olympus will continue to monitor field performance for this device.
