Event Description
|
An attempt was made to use two nc euphora rx balloon catheters to treat a lesion.The devices were inspected with no issues noted.Negative prep was not performed on the 4.0x20mm nc euphora balloon catheter.Negative prep was performed on the 4.0x15mm nc euphora balloon catheter with no issues noted.It was reported that both nc euphora balloon catheters would not deflate at the lesion site.It was detailed that both balloon catheters were damaged, one balloon catheter the tip was kinked and the other balloon catheter was bent.The patient is alive with no injury.
