Event Description
It was reported that during the implant procedure, the catheter hemostatic valve had a suspected failure.The physician was concerned about the reverse blood from the hemostasis valve when cannulation was performed with the catheter.It was noted that contrast study without the valve tool was attempted.The catheter was replaced.No patient complications have been reported as a result of this event.
 
Event Description
It was also reported that the hemostasis valve was noted with leakage.The cause of the issue was unidentified.
