The product has not returned for analysis, however, pictures were provided by the customer.Evaluation is still in progress.The analysis has begun but is not completed at this time.When the investigational analysis has been completed, a supplemental 3500a report will be submitted.This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by biosense webster inc., or its employees that the report constitutes an admission that the product, biosense webster inc., or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).
 
Event Description
It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a thermocool® smart touch® sf uni-directional navigation catheter and a presence of a gel-like substance in the sterile catheter packaging and catheter was found.Presence of a gel-like substance in the sterile catheter packaging, as well as on the catheter at the tubing connection.This presence was noted just as the catheter was about to be given to the doctor by the ward nurse.The packaging appeared to be intact before it was opened.The catheter was not used.Use of another device to complete the procedure.No surgical delay.No patient consequence.The substance was described as a gelatinous, viscous, thick, transparent substance.

On 8-nov-2023, the bwi product analysis lab received the complaint device for evaluation.The product analysis has begun but is not completed at this time.When the investigational analysis has been completed, a supplemental 3500a report will be submitted.Additionally, the manufactured date and expiration date have been provided.Therefore, fields d4.Expiration date and h4.Device manufacture date have been populated.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).

It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a thermocool® smart touch® sf uni-directional navigation catheter and a presence of a gel-like substance in the sterile catheter packaging and catheter was found.Presence of a gel-like substance in the sterile catheter packaging, as well as on the catheter at the tubing connection.This presence was noted just as the catheter was about to be given to the doctor by the ward nurse.The packaging appeared to be intact before it was opened.The catheter was not used.Device evaluation details: the product was returned to biosense webster inc (bwi) for evaluation and the evaluation has been completed.A visual inspection of the returned device was performed following bwi procedures.The device was inspected, and no physical damage was observed.According to the photo provided by the customer, foreign material like-oil was observed into the sterilization package; however, the package was not returned for analysis.Based on the photo provided by the customer, a manufacturing meeting was performed and the investigation results determined the customer complaint is not confirmed to be related to manufacturing.It is important to state that transparent matter reported by the customer is not used at packaging area and the physical sample was not received.A manufacturing record evaluation was performed for the finished device batch number, and no non-conformances were identified.The issue reported by the customer was confirmed based on the pictures provided by the customer.As part of biosense webster's quality process, all devices are manufactured, inspected, and released to approved specifications.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).
