Event Description
The customer reported to olympus that during an inspection for use, the evis exera iii xenon light source had no image, displayed errors b30 and e216, and presumably had moisture in the socket.There were no reports of patient/user harm.

The device was evaluated by an olympus technician onsite, the customer's allegation was not confirmed.The technician confirmed that the light source had no problem, but the endoscopes were all full of water.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or when additional information becomes available.

Corrected data: d9 (¿no¿ was selected in error on the initial report), h3 (¿no¿ and ¿not returned to manufacturer¿ were selected in error on the initial report), and h6 (type of investigation code ¿4114¿ was selected in error on the initial report instead of ¿10¿) this report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 7 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the b30 error and water penetrating into the power plug of the device could not be identified.However, it¿s likely the cause of the b30 error is related to a scope communication error.Olympus will continue to monitor field performance for this device.
