Event Description
Information was received from a healthcare provider via a manufacturer representative regarding a patient receiving unknown morphine (10mg/ml, dose decreased from 3.575mg/day to 1mg/day) via an implantable pump.The indications for use were unknown.It was reported that there was a 35ml residual on the last refill.The healthcare provider (hcp) attempted a dye study, but was unable to aspirate the catheter.A rotor study was performed and was normal.The pump was flipped with the connector to the left on plain x-ray.They were unable to interrogate the pump until it was manually flipped to the correct orientation by the hcp.There were no known environmental, external, or patient factors that may have led or contributed to the issue.Surgical intervention was planned but had not been scheduled.The issue was not resolved at the time of the report.It was noted that the hcp had no further information.No patient symptoms were reported.The patient's medical history included chronic pain.The patient status at the time of the report was alive with no injury.

Additional information was received from a healthcare provider (hcp) via a company representative (rep) stated that the pump segment of the catheter was revised on (b)(6) 2023 and the issue was resolved.
