The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure.There is no indication of a product quality issue with respect to manufacture, design or labeling.
 
Event Description
It was reported that the procedure was to treat a lesion in the middle third of the descending artery with mild calcification and tortuosity.The 2.75x18mm xience sierra stent delivery system (sds) was being advanced with resistance through the 6french (fr) guide catheter when the devices got tangled.When trying to remove the system, the stent dislodged from the balloon and became stuck in the guide catheter.The stent was removed together with the guide catheter.Another non-abbott stent was used to complete the procedure.There was no adverse patient effect and there was no clinically significant delay in the procedure.No additional information was provided.
