Event Description
According to the reporter, during a stoma reversal procedure when performing a end to end anastomosis, the knife blade still cut, but no staples were fired, the staples were missing.The staple site was disrupted after the removal of the stapler as the anvil could not be tilted.The anastomosis was hand sewn after the removal of the anvil to resolve the issue.The surgical time was extended 30 minutes or more due to the product problem.
