To date, the device has not been returned to olympus for evaluation.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, there was a brightness problem - dark image while using the light source during a diagnostic cystoscopy procedure.The procedure was completed using the same device.As reported, there was no impact or harm to the patient.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the device evaluation and the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over nine (9) years since the subject device was manufactured.The device was returned to olympus for inspection, and the customer's complaint was confirmed.In addition, the following non-reportable malfunctions were found during the device evaluation: rust on the bottom chassis and rear leg and high brightness mode was used intermittently due to wear of the socket slider switch.A definitive root cause was not identified, however based on the results of the investigation, the probable cause of all the malfunctions would likely be a defect in the optical lens.Olympus will continue to monitor field performance for this device.
