Event Description
The customer reported to olympus during preparation for use for a diagnostic gastrointestinal procedure, the light source was too bright.There were no reports of patient harm.
 
Manufacturer Narrative
The customer was provided with the phone troubleshooting by the olympus sales representative.The customer was asked to swap out the scope with a known good working one from another room and tried to shock the chip with red, white balance but had no effect.The device was returned to olympus for evaluation and the customer's allegation was not confirmed.Additionally, the following was found during evaluation: scratches on the top cover and dents and a fracture on the front panel mouth.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Based on the results of the investigation, the root cause for the event could not be determined.Three attempts were made to obtain additional information regarding the reported event, but no response was received from the customer.Olympus will continue to monitor field performance for this device.
