Additional information was received: fault did not occur during patient use and no patient injury.

Other, other text: additional information: a1, b5, d4.Udi, h3.Device evaluated by manufacturer and h6.Health effects codes, evaluation codes: updated.D3, g1,2 email is: (b)(6) one device was received.Per visual inspection, loud pump, printed circuit board (pcb) with broken light emitting diodes (led)s, bent micro switch, and broken front cover.Upon removing the front cover there were broken and missing leds on the pcb confirming the complaint.The root cause was removal or installation of the front cover incorrectly (as the font cover was also broken), a broken pcb board led cap from improper cover installation by the user.The product's history records were reviewed and there were no non-conformances nor service-related issues that would have resulted in the reported complaint.Replaced the pump, pcb, micro switch, and front cover.Performed preventative maintenance (pm).The device passed all functional and delivery tests.
