(b)(4).G2: foreign- south africa.Customer has indicated that the product will not be returned to zimmer biomet for investigation, as the device location is unknown.The investigation is in process.Once the investigation has been completed, a follow-up mdr will be submitted.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
 
Event Description
It was reported that the head of the impactor broke and a piece got stuck inside the tibial targeting guide.There was a surgical delay of ten minutes.Attempts have been made and no further information has been provided.No adverse event has been reported as a result of the malfunction.

Reported event was unable to be confirmed due to limited information received from the customer.Device history record (dhr) review was unable to be performed as the part and lot number of the device involved in the event is unknown.Root cause was unable to be determined as the necessary information to adequately investigate the reported event was not provided.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.

This follow-up report is being submitted to relay additional information.Reported event was confirmed by review of pictures.Visual examination of the provided pictures identified a targeting guide handle which shows wear and tear from repeated use. one of the holes on the targeting guide appears to be blocked.Device history record (dhr) review was unable to be performed as the part and lot number of the device involved in the event is unknown.Root cause was unable to be determined as the necessary information to adequately investigate the reported event was not provided.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
