To date, the customer have not returned the device for evaluation.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus that videoscope cable exera ii, had video connection issue.Connector it will not align it is turning inside the sleeve.The issue occurred the preparation for use.The procedure was diagnostic.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegations was confirmed.The evaluation found that the flickering/intermittent image occurred due to the faulty cable, and the connector would not align, and it is turning inside the sleeve, a bad scope connector was found damaged round the connector moving from the cover.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

Correction: e2 and e3 were inadvertently omitted from the initial report.This report is being supplemented to provide additional information, based on the legal manufacturer's final investigation.A review of the device history record found no deviations, that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the event was unable to be identified.The event likely, occurred, due to a cable failure.Olympus will continue to monitor field performance for this device.
