Event Description
Medtronic received information that during the implant of this 23mm aortic bioprosthetic valve, it was replaced with a 21mm valve of the same model.The reason for the replacement was reported as an occluded coronary artery due to the valve being too large in size.It was indicated that the size was selected as per the instructions for use (ifu) using the sizer.No additional adverse patient effects were reported.

Event Description
Medtronic received additional information which stated that the valve had been sized using the valves corresponding sizer.It was stated that both the barrel end and replica end of the sizer were used and the replica end of the sizer was used to select the size of the implanted valve.It was reported that the annulus was felt to be between two different valve sizes of 21mm and 23mm.It was noted that it was the preference of this physician to upsize the valve for a larger effective orifice area.It was mentioned that the body surface area (bsa) chart indicated that a larger valve was needed than what was sized for this patient.A root enlargement was not performed.It was stated that the valve was not re-sized following the pledget placement.No additional adverse patient effects were reported.
