Event Description
|
According to the reporter, during lap anterior resection, the surgeon had been using the instrument to dissect vessels and then when they were dissecting the meso rectum they noticed sliding mechanism had stuck and they could not retract the hook or advance the device.The surgeon removed instrument from port and noticed insulation at proximal part of instrument at the handle was detached and sliding back and forwards exposing metal shaft underneath.The procedure was completed with another device.There was no patient injury.
