Event Description
|
It was reported the arterial patient connector was observed cracked on a gambro cartridge dn prime line.When the patient arrived and was hooked up for the start of the treatment, the nurse noted that the arterial pressure on the machine was running quite high.The nurse attempted to reverse the lines on the machine's tubing to the patient catheter; however, the arterial pressures remained high.A second nurse came in to help troubleshoot and they were going to attempt to flush the patient catheter to assess if they felt any resistance that would note a clot/kink in the patient catheter that could be causing the high pressures.Once the arterial side of the machine tubing was disconnected from the patient catheter, the red lumen that helps twist on to the patient catheter "broke off" from the machine tubing and stayed on the patient catheter.They were able to remove the piece from the catheter and get it back on the tubing to be able to return the patient's blood.The treatment was ended, and a new system was set up to begin treatment with supplies.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
